Community Empowerment Network

Community Empowerment Network (CEN, formerly Wandsworth CEN) are a local group dedicated to eradicating healthcare inequalities. They work with communities to co-create solutions that address ethnic disparities. We have partnered with them on various pieces of work.

We are currently working with them to develop the Battersea Community Health and Wellbeing Worker Project.

Enable

Enable is a not-for-profit organisation, working in partnership with councils, charities and other organisations to deliver health, leisure and community services and events that enrich people’s lives.

We partner with them in a number of different ways. At present, they provide our Health and Wellbeing Coaching and Social Prescribing Services. We are also collaborating with them to develop our Battersea Community Health and Wellbeing Worker Project.

South West London and St. George’s Mental Health NHS Trust

South West London and St George’s Mental Health NHS Trust are the leading provider of mental health services across South West London. We employ our adult and young person’s assistant psychologists together with them. This allows for greater understanding and collaboration between Primary Care and Mental Health NHS organisations, so that people are better supported with their mental health.

South West London Integrated Care System

In Wandsworth, we are part of the South West London Integrated Care System (ICS). This is a partnership of primary care, hospital, social care, mental health and voluntary and community health and care services.

As they are such a large organisation, our work with the ICS is key to everything we do.

In particular, it is only with the support of the ICS that we have been able to develop the Community Health and Wellbeing Worker project. They have championed and developed this alongside us and other partners.

Surrey Physio

Surrey Physio organisation which provides us with First Contact Practitioners (FCPs) who can diagnose and manage musculoskeletal conditions.

They provider our FCP service alongside St. George’s University Hospitals NHS Foundation Trust.

St. George’s University Hospitals NHS Foundation Trust

St. George’s University Hospitals NHS Foundation is a very large hospital near our PCN. They provide a huge variety of services, and are a large organisation employing over 9,000 staff. You can read more about them here, https://www.stgeorges.nhs.uk/about/

They are one of 2 organisations that we use to provide our First Contact Physiotherapy Service. This helps people get the right care, both at our GP surgeries, but also if they need onward referral to local physiotherapy (all provided by St. George’s University Hospitals NHS Foundation), or other specialist input.

Wandsworth Council (including Public Health)

Wandsworth Council is the London Borough Council for the majority of people that we look after (though some also live in other boroughs, in Lambeth, for example). They are responsible for organising Social Care, as well as for Public Health.

The main area where we are currently working together is with Wandsworth Public Health in the development of our Community Health and Wellbeing Worker Project.
